Gold Card
To participate in our athletics programs, all middle school and high school student athletes must have the completed forms listed below on file in the school office in order to receive a Gold Card. When the student athlete comes into the office, the receptionist will confirm whether or not all necessary forms are in his/her file and that the previous sport uniform and equipment have been turned in. If so, the receptionist will issue a Gold Card.
The Gold Card is then given by the student athlete to his/her coach, which qualifies the student to participate in practices and games. Students may not participate, even in practices, until the Gold Card has been given to the coach.
The Athletic Fee is $100 per sport season for all sports, other than football, which is $150. This fee will be included on the student’s monthly invoice mailed to parents once the team rosters are finalized.
- Yearly Physical Examination: This physical must have been completed by the doctor after March 1 and submitted to the office before August 1. Click here for the Physical Examination Form if you do not already have one. Most families already have this form either from the Re-enrollment Packet, or the Application/Enrollment Packet if you are a new family.
- Medical/Emergency and Parental Consent Form (completed and signed): Click here for the Medical/Emergency and Parental Consent Form. Most re-enrolling students and new students will have this completed form in their file as it is required to be submitted as part of the Re-enrollment and the Application/Enrollment process.
- Birth Certificate: Again, most students have a birth certificate in their files. If not, the parent needs to send in a copy before the Gold Card will be issued.
- Concussion (Brainbook) Education Certificate: Complete this state required course online at www.AIAOnline.org. See the athletic department for questions and more information.
